Confirmed Eligibility:
- Existing LPG connections in Telangana with a history of usage. This means you must have actively refilled your cylinder at least once in the past.
Unconfirmed Eligibility:
- New LPG connections: Initial reports suggested these would be included, but recent updates from the Civil Supplies Department indicate they might be excluded.

- Proof of identity: Aadhaar card, Voter ID card, PAN card, etc.
- Proof of address: Aadhaar card, electricity bill, ration card, etc.
- Existing LPG connection Proof: LPG booklet or a document from your LPG distributor.

1. Download & Fill Applicant Information:
- Download the application form – Click Here
- On the first page, carefully fill in your personal details and family information. Ensure all information is accurate and complete.
2. Select Mahalakshmi Scheme:
- Go to the second page of the application form.
- Locate the Mahalakshmi scheme section.
- In the second row (“మహాలక్ష్మి పథకం” in Telugu), tick the “Yes” box.
3. Update Gas Information:
- In the remaining three rows provided for the Mahalakshmi scheme, enter the following details:
- Gas Receipt number: Enter the receipt number from your recent gas purchase.
- Gas Connection number: Provide your unique gas connection number.
- Provider name: Indicate the name of your gas supplier company.
4. Sign & Submit:
- Proceed to the last page of the application form.
- Sign on the designated line, providing your full name for verification.
- Submit the completed and signed application form to the gazetted officer at your local Mandal Panchayat office.
